Abstract
Background: Vitiligo is a chronic depigmentation disorder affecting 1-3% of population. Topical steroids/ immunomodulators, phototherapy and various surgical grafting procedures are used with variable success in the treatment of vitiligo. Newer treatment modalities include the 308nm excimer laser. Aims: We conducted an open study to determine the efficacy of the Xtrac? 308nm excimer laser (Photomedex, Radnor, PA) in the treatment of vitiligo affecting Asian patients with Fitzpatrick skin types IV and V. Methodology: This is a prospective non-comparative open study. Laser treatment was performed on the vitiligo lesions twice per week for 6 months. Outcome was assessed via photographs taken before treatment and at the end of 6 monthsÌ treatment. The degree of repigmentation was graded as <25% (poor), 25 to 50% (moderate), 51 to 75% (good) and >75% (excellent). Results: 49 vitiligo spots in 35 patients were evaluated. The treatment was found to be moderately effective and body-site dependent. Better results were obtained from treating non-segmental facial lesions, truncal lesions and lesions on proximal limbs. Lesions on distal limbs responded poorly to treatment. Side effects (erythema and superficial blistering) were mild and transient. Conclusions: The excimer laser is effective in the treatment of limited and recalcitrant vitiligo. However, the best treatment regimen for these patients, especially those with treatment resistant sites, requires further evaluation
